Working with the San Jose Area Medical Community

 

San Jose has the largest senior population on the Peninsula with over 75,000 residents aged 65 or over. These numbers are expected to increase at least 50% over the next 20 years. With over 90% of San Jose seniors intending to age at home, Homecare California is rapidly expanding to serve this community with in-home care services.

We work closely with medical personnel from local hospitals like O'Connor and Good Samaritan as well as skilled nursing facilities such as Almaden Health & Rehab Center and Mission Skilled Nursing for safe discharges from hospital to home.  After the successful transition home, we provide in-home care services from experienced caregivers and Certified Nurse Assistants.

In addition to providing care in the home, we encourage our clients to participate in any of the various community activities that San Jose has to offer.
