Medication Management

The average senior takes 14 different kinds of medication. It's no wonder that medication mismanagement is a major cause of unplanned hospitalization.

Homecare California clients have access to private registered nurses that can provide medication setup and ongoing management under their own nursing licenses.

 

These services can include:

  • Analysis of existing medications and dosage levels

  • Medicine or dosage modification communication with physicians

  • Setting up medicine in weekly/monthly cassettes.

  • Periodic monitoring, adjustment, and refilling of medication

  • Analysis of medicine ordering and billing processes.

  • One way or round trip

  • Regular or occasional transportation needs

  • Wheel chair bound transportation available

  • Discharge home from hospital or nursing facility

Even after our caregivers go home, Homecare California is still there.  For qualifying clients, we provide free 24/7 emergency personal response service for our clients through Philips Lifeline, the most trusted name in the business.  With a touch of a button on a bracelet or pendant, seniors can get help when they most need it.

There are no installation fees, no long-term commitments.  If you no longer utilize our home care services, you can always continue your Lifeline service by simply picking up the cost of the monthly service which you can cancel at any time.

Physical, Occupational & Speech Therapies at Home

Many of our clients prefer to receive therapy in the comfort of their own home rather than traveling to a rehab facility.

Homecare California works closely with therapy professionals that provide these services in-home and can work closely with our care managers and caregivers to support the programs they put in place.
