Do you like variety in your day, being out in the community and interfacing with older adults, family members and medical professionals as well as your company teammates? Would you like working closely with families to implement care solutions that enable their older adult loved ones to age at home? Do you have the capacity to communicate and quickly gain trust over the phone and in person with a variety of personalities and people ranging from 20 to 100 years old? In one of our most important roles as a Client Care Manager, you will work with an existing base of home care clients and their families, work with prospective clients to implement solutions and interface with case managers and discharge personnel at skilled nursing facilities, hospitals, physicians, etc.. Best of all you will be helping families put long term care solutions in place for seniors to promote independence, improve their quality of life and provide peace of mind for those that love them.

Responsibilities

Work with 30-40 current senior clients living at home to make sure they have the best care possible. You will communicate with family members and their caregiver team to refine and improve their lives. You will ensure high client satisfaction based upon key measurement criteria.

Work with our intake coordinators and/or take inquiries from prospective clients to set up and perform in home assessments that lead to solutions which allow them to age gracefully at home.

Work with and develop trusted referral relationships with key professionals at skilled nursing facilities, hospitals, geriatric care managers, etc.

Create care plans and work with your internal care manager counterpart to put the right caregiver team in place. Perform client drop-in visits, update care plans and suggest changes in care as needed.

Provide guidance to caregiver teams as a team leader and coach.

Identify and communicate solutions beyond caregiving that may be beneficial for the health and well being of your clients.

The ideal candidate will have the following qualifications:
-
A demonstrated interest in helping people in a health related field. Those disciplines may include experience and education in health administration, social work, behavioral therapy, nursing, occupational or speech therapy, medical assistant or any discipline helping seniors and communicating with family members.

Ability to work as an advocate and leader in a team environment on behalf of clients.

Understanding how successfully work with key referral sources

Ability to use technology tools for communications, documentation, etc.

Ability to be effective working independently in the field and and effective in marshalling needed resources from your co-workers that support your success.

Ability to effectively interact with all persons of all ages and backgrounds.

Team player that adds to the company culture and performance of the overall team through trust, competence and enthusiasm.

Willingness and an eagerness to learn and grow.
