Working with the Los Gatos Area Medical Community

 

The residents that live in Los Gatos love their community.  Many have lived in the area for decades and as they age, and over 90% of the older adults in Los Gatos will want to live at home as they age.

We work closely with medical personnel from local hospitals like El Camino Los Gatos and Good Samaritan as well as skilled nursing facilities such as Vasona Creek and Los Gatos Meadows skilled nursing for safe discharges from hospital to home. After the successful transition home, we provide in-home care services from experienced caregivers and Certified Nurse Assistants.

 

In addition to providing care in the home, we encourage our clients to participate in any of the various community activities that Los Gatos has to offer.
